چکیده

Objective: To evaluate the utility of inspiratory capacity (IC) measured by simple spirometry as an indirect measure of air trapping. Method: Functional residual capacity (FRC) and IC were determined, respectively, by a body plethysmography and by spirometry. Air trapping was defi ned by a FRC higher than 120%. Spearman’s rho between IC and FRC is determined and the receiver operating characteristic (ROC curve) is calculated as predictive of air trapping for IC. The best cutoff of the overall test is calculated. Results: 148 patients were included with a mean FEV1 60% (SD 20%). There was signifi cant correlation between IC and FRC with a Spearman rho -0.673 (p <0.0001). The area under the ROC curve for IC and FRC was 0.803 (95% CI 0.709 to 0.897). The optimal cutoff for predicting air trapping was given for a forced IC less than 80% of its theoretical value, with a sensitivity of 82%, a specifi city of 73%, a positive predictive value of 73% and a negative predictive value 82%. Conclusions: Inspiratory capacity is useful in the assessment of air trapping and may determine a cut below 80%. CT Air Trapping Is Independently Associated with Lung Function Reduction over Time

PURPOSE We aimed to study the association between lung function decline and quantitative computed tomography (CT) air trapping. MATERIALS AND METHODS Current and former heavy smokers in a lung cancer screening trial underwent volumetric low-dose CT in inspiration and expiration. Spirometry was obtained at baseline and after 3 years. The expiratory to inspiratory ratio of mean lung density (E/...

Risk of Lung Cancer Associated with COPD Phenotype Based on Quantitative Image Analysis.

BACKGROUND Chronic obstructive pulmonary disease (COPD) is a risk factor for lung cancer. This study evaluates alternative measures of COPD based on spirometry and quantitative image analysis to better define a phenotype that predicts lung cancer risk. METHODS A total of 341 lung cancer cases and 752 volunteer controls, ages 21 to 89 years, participated in a structured interview, standardized...

R-V graph in whole body plethysmography

Slow spirometry and forced spirometry (flow volume loop) are often used to measure clinically relevant lung volumes and forced expiratory and inspiratory flows. Dynamic parameters, such as forced expiratory volumes in one second (FEV1) or forced vital capacity (FVC) and static lung volumes, such as expiratory reserve volume (ERV), inspiratory capacity (IC) and inspiratory vital capacity (IVC) c...

Paired inspiratory-expiratory chest CT scans to assess for small airways disease in COPD

BACKGROUND Gas trapping quantified on chest CT scans has been proposed as a surrogate for small airway disease in COPD. We sought to determine if measurements using paired inspiratory and expiratory CT scans may be better able to separate gas trapping due to emphysema from gas trapping due to small airway disease. METHODS Smokers with and without COPD from the COPDGene Study underwent inspira...

Assessment of Small Airways with Computed Tomography: Mosaic Attenuation or Lung Density?

BACKGROUND Computed tomography (CT) assessment of air trapping has been considered useful as a measure of small airway disease. Mean lung density (MLD) and the percentage of the lung field occupied by low attenuation area (LAA%) can be evaluated automatically, and their expiratory/inspiratory (E/I) ratios correlate with asthma severity and spirometry parameters. However, mosaic attenuation, ano...
